Supplementary application form for those seeking a community place in

You must complete a Common Application Form, which you will obtain from your local
LEA and you must return it to the LEA. IN ADDITION, you must complete this form, for entry to Saint Paul's
Academy.
If there is any additional information that you wish to provide, please include it in section 6.
If you do not provide all relevant information, this may affect the admissions selection process. If you
make any false statement upon this application, any place offered will be withdrawn. If you have any
difficulty in completing this form, please contact the Academy Registrar on

Please note that, if your child has a statement of Special Educational Needs, you will NOT be able to
apply directly online since evidence of medical, social or special educational needs must be submitted
